This includes diagnosis, prognosis, and guiding treatment decisions. Aside from the important measurements of left ventricular (LV) volumes and ejection fraction (EF), Doppler can be successfully applied to assess LV and right ventricular hemodynamic status. Several studies have shown the accuracy of these techniques against the invasive gold standard (1–12). In fact, Doppler echocardiography provides data that are comparable to that obtained by a pulmonary artery (PA) catheter (13).

Heart failure with normal left ventricular ejection fraction: what is the evidence?
Heart failure with a normal ejection fraction (HFNEF) is a common clinical problem with many unsolved questions regarding pathophysiology, diagnosis, and therapy. Although the term diastolic heart failure has been abandoned, diastolic left ventricular (LV) dysfunction together with combined systolic ventricular and arterial stiffening are considered to be the main pathophysiologic mechanisms in...
متن کاملDoppler tissue imaging: a noninvasive technique for evaluation of left ventricular relaxation and estimation of filling pressures.
OBJECTIVES This investigation was designed 1) to assess whether the early diastolic velocity of the mitral annulus (Ea) obtained with Doppler tissue imaging (DTI) behaves as a preload-independent index of left ventricular (LV) relaxation; and 2) to evaluate the relation of the mitral E/Ea ratio to LV filling pressures. BACKGROUND Recent observations suggest that Ea is an index of LV relaxatio...

BACKGROUND Invasive studies have shown that cardiac resynchronization therapy (CRT) acutely improves left ventricular (LV) systolic performance and lowers filling pressures in a majority of patients with medically-refractory severe heart failure. Measurements included LV volume, ejection fraction, PWD early (E-wave) and atrial (A-wave) velocities, diastolic filling time (DFT), and DTI early dia...
متن کاملReducing cardiac filling pressure lowers norepinephrine spillover in patients with chronic heart failure.
BACKGROUND We studied the cardiac sympathetic response to selective unloading of cardiopulmonary baroreceptors in subjects with normal left ventricular (LV) function and congestive heart failure (CHF). METHODS AND RESULTS Eight patients with normal LV function (age 57+/-5 years, ejection fraction 58+/-2%) and 8 patients with CHF (age 60+/-2 years; ejection fraction 19+/-2%) were studied. Inst...
متن کاملComparison of the effect of pressure loading on left ventricular size, systolic and diastolic function in canines with left ventricular dysfunction with preserved and reduced ejection fraction
BACKGROUND Decompensated heart failure may present with severe hypertension in patients with preserved (PreEF) or reduced left ventricular (LV) ejection fraction (RedEF) and is clinically indistinguishable. Previously, we demonstrated that arterial pressure elevation increases LV filling pressures in a canine model of chronic LV dysfunction with PreEF or RedEF. It is not clear whether any diffe...
